I assigned a moderator, I would like all posts to be approved before being live. Moderator seems to be assigned but I don't know how to make Hold active. Spammers are signing up bogus accounts and posting spam on my board. Thanks!

There are many posts about stopping spammers. Try the search page.

Regarding the moderation feature, you need to set Topic Moderation to On in Admin Options-> Feature Configuration. Then you need to edit each forum's properties and set the moderation level to the one it suits you. IMO, using some of the strategies to stop spammers (first line being Email Validation and Restrict Registration) you should not need to turn post moderation on.
