I'm trying to install the IP Ban Mod on Schoolofduel:

www.schoolofduel.com

However the forum version is Snitz Forums 2000 Version 3.4.03. I need the IP Mod for that forum version please.

The links Podge provided for IPGate is for v3.4 and will work for your forum. You might have to do some changes on other line-numbers than mentioned in the readme, but that is all. Im not sure if Davio or anyone else has updated the Ban User IP Mod to the current version, but IPGate does the same thing.
