Is there a way to avoid this sort of login names? Its the same member. Only the login is case sensitive, the database not.

That is a weird situation, that happens sometimes, but which is hard to reproduce (I can't). As you correctly pointed out, the name is treated by the database as a case insensitive name, so something else caused it, in a set of circumstances which I label as abnormal. That normally won't happen and since I don't know how it happens, it's impossible to prevent.
The image you posted shows that the user is being "recognized" twice, that is, each visit, each post is being recorded to both users, which shows indeed that in the current conditions, that wouldn't happen again.

I agree that it is hard to find, especially in my forum that is moded for the last 5 years.

PS: I notesd that my jpg posted above as a url is giving a funny output, still there is noting strange in my post? Bin looking in my forum for some time, now I see its a general bug?

That's because you have a semicolon after the http in the URL where you should have a colon.
