Does anybody know of any major accidents or incidents where the main cause was attributed to poor interface design? Such as where the operator have wrongly operated switches, etc and made a bad situation worse. I am thinking along the lines of aircraft, powerstations, and so forth. Was the accident at three mile island to do with this?
I need to make a case study about this and present it to a group. |

Some years ago I seem to recall someone got massively x-rayed because the ui let the operator do something stupid.

Hi, chumbawumba.
The three mile island accident was caused by a series of factors, among which we could also think of a misinterpretation of the pressure valve's situation, I think.

There is also some information about three mile, and others. Sorry, I am at the office so I cannot filter the reults anymore for you, but at least I hope I have given you a useful clue |

Hi Mikel I am slowly finding some relevant information about what i need thanks ! One of them is about the aeroplane accident, and another one about an aeroplane that was erroneously shot down. Thanks!

Sorry, I don't. My faulty memory says it was from an early computer-controlled xray machine.
OK, google showed up this article, I think it's the same thing I was thinking of. From the article it was more a programming error.

try the ergonomics section of a library, usually have a load of case studies in textbooks.
