I could not find any posts that anyone else has this problem so here it is. I hope one of you can help me out.

Recently my hosting company moved me to a new server and since then my forum has been acting strange. I worked through most of the database errors and such but the newest problem i can not figure out. The user name and password boxes have dissappeared from the screen also the logon button is missing. How do i get them back with out re-installing?

I modifyed the code to add a dice roller and really do not want to reinstall because of that. It is not the dice roller that is causing my trouble since it ran for a year or better on the old server with that code in it. My site address is www.wizardsrepose.com/holder/forum

have you run setup.asp since the forum was moved? If not, try running it.

I get a 404 error when trying to visit your site (http://www.wizardsrepose.com/holder/forum)

Sorry they moved me again and i just uploaded the site again. I am still having the same trouble please check it out again. I did run setup.asp.

looks like you have NT Authorization enabled, you need to change it to DB.

Admin Options --> Main Forum Configuration --> Authorization Type

I can not get in to admin options. Is there some place it is stored in the database? If so what do i change it to?

As said by Richard, you needed to run setup.asp

I did it for you, as you kinda ignored him.

I tried running it every time i did it just said it was done successfully. Anyway i found the entry in the database and fixed it. But as I said i did run setup but every time i did it took me straight to the ‘you have successfully completed setup’ page.

That's what it should say, you did it correctly (running setup.asp).
