Hi, I am building a survey app which I hope to sell commercially.
Now, as you know, there exists such a thing as "survey fatigue", the groans and yawns that people give when they are constantly required to register / sign in to things. Therefore I am giving special consideration to the styling of my survey forms. It needs to look slick, professional and user-friendly.
My survey is made up of 5 sections comprising only radio buttons and dropdown lists (and a couple of free text fields).
Using CSS or an innovative layout, has anyone seen a survey form recently that looks good ? Any examples much appreciated !

I've been doing some beta testing for an online community (There) and they use Zoomerang for their post-test surveys. Nothing fancy, but they use images for checkboxes and radio buttons. It looks nice, but even at my connection speed its reactions are slow when you make a selection. Unless you do it correctly by preloading the images, I'd advise you not to do what Zoomerang has done.
Other than that, all I can say is keep the pages short, with few questions, and give the ability to look back without losing the choices made when you go forward again. I hate filling in a survey, going back a few questions 'cause I changed my mind, only to find out the answers of the questions I'd answered have been erased. |

Thanks for the info about Zoomerang. I hadn't heard of it before. Having built the application from scratch I know that it is both complex and time-consuming. They have some screenshots that look interesting.

My design takes this into account. but it's a pain !

Maybe a pain to create the surveys to work like that, but it's heaven for the users :P |
