 Not showing Avatar Mod for 3.4 in the dropdown l

I have problem in implementation of Avatar mod.
I have uploaded zip file of :
Avatar Mod for 3.4

and have modified all the files I found in readme.html in that zip.
And all the modified files are saved in my computer (not uploaded yet)

I can't see that dropdown option in my mod setup.

Although readme file is showing the steps. I followed all.

1 ) Upload all the files to your forum directory.

2) Update the database

3) Go to Admin Options > Mod Setup
4) Select Avatar Mod for 3.4 from the dropdown list
5) Click "Update"
6) After the database has been updated you can delete the dbs_avatar.asp file.

I am stucked at 4th step. Please tell me where I am doing mistake. Thanks in advance.

When you can't use MOD Setup, you can do the following:
- Open dbs_avatar.asp in a text editor
- copy its content
- go to "Alternative MOD Setup"
- paste the code into the form
- click "Submit"
