Since my forums are all private, members only, I need a "Contact Us" email form for visitors to use to contact me without having to register.

Has someone created one that you'd share or does anyone have a link to a possible good one on the web? Just something very basic, yet professional looking, is all I need so I can place a link to it for visitors.

The page allows anyone to visit it. They do not need to belong to your forum. If your forum is private, then you will need to place a link on your register page for the contact form.

It does give a page with confirmation that the message was sent.
