I am looking for a sample code to use for on-line form as follows: 1) After the users fill in the form and press the "Submit" button they will get a screen showing them what they have entered. 2) If they agree they can send again where the information will go into a databse. 3) If they do not agree and decided to either cancel or edit another button will take them back the original form.
