Yep, that's exactly what I was looking for. I looked briefly, then decided to quit being lazy and build it myself. I didn't go overboard with ambition though, so it doesn't use a database. I just used Dreamweaver to build the form in html and hardcoded the email (user names only) into the options list. I didn't use full addresses to avoid spam. I then have an ASP sendmail file that grabs the server name and pairs it with user name to compile a complete email address and sends the message.
It works like a charm, and staff email adresses are never exposed for harvesting.
