On occasion, on certain computers, when I open my browser (which has my active topics page as a home page-tab), the "last visit" time changes when the page refreshes. One of my users is complaining about the same problem. Its happens on both browsers. Unfortunately, it doesn't happen on my home computer so I can't re-create the problem.

Has anyone observed this? Is it a cookie problem?

Did they spend 20+ min. before they refreshed? Usually I've seen that happen when they spend too much time reading a topic.

Is refreshing with the refresh button/F5 synonymous with letting the active topics page refresh on its own?

My user had the problem after 20 min.

When I encountered the problems it was on the first automatic refresh of the page even if its just seconds after I first open the browser.

Then their session had expired and a new session was initiated for them, updating their last visited time.

No. If you wait more than 20 min. the session will be considered expired, etc. like Shaggy said.

As to this one, I couldn't tell you what happened...
