I run Snitz Forums 2000 Version 3.4.03 SpeedBall, and have found that sometimes new registrations appear in the pending members list and sometimes they do not and just register and get activated - why is this?
I have email validation on and restrict registration off
Sorry, what's the problem exactly? If an account has been activated, it will no longer show up on the pending members page.

First: Am I correct in understanding that the problem/question is that some people seem to just sit in the pending members list and some don't - and you're not sure why?
Assuming that the e-mail validation is working it could be that:
- the people in the pending members list didn't get the e-mail (mailbox could be full, it was caught in their spam filters, they don't check their e-mail, ad nauseam)
- its a spambot and will never validate the e-mail I've had a few of those lately.
But without more detail, its hard to say. Are all of your forum's e-mail functions working properly?
