I have sucessfully installed the Avatar mod and the My_Avatar add-on on my version 3.4.05 forum : http://www.pamelamoore.com/forum

Recently, I wanted to make one of my members a moderator. So I logged in as Admin and tried to modify this member's profile. However... after pressing the "submit" button I got a page with this error:

Microsoft JET Database Engine error '80040e14'

Duplicate output destination 'M_AVATAR_URL'.

/forum/pop_profile.asp, line 1572

Can anyone help me?

That means you repeated the addition of the field M_AVATAR_URL to the existing code. I would suggest you make sure you follow the Avatar mod's instructions regarding the addition of the M_AVATAR_URL field somewhere near line 1572, pop_profile.asp.
