The avatar mod seems to work only speratically. I have a listing of images to use for avatars (some reservered for certain users). When they select the avatar it is display in their profile, however, when posting the avatar infrequently displays.

I'd suggest it's your implementation. There are many sites that have been using this MOD for years - no ill effects at all.

When the avatar image doesn't display, what happens if you right-click on it and chooses Show Picture?

When the image does not display in the post (even though it's in the profile) there isn't even a place holder (broken image link).

It looks from here http://www.jasonknox.ca/messageboard/topic.asp?TOPIC_ID=3 that you've missed some changes to topic.asp (assuming you want your avatar in the reply posts). I can't find any broken avatar links though Have you got a link for that ?
