I've modified it to work in a similar way on my site; here's a stripped down version of the code I use (you'll also need the upload.asp file from the original download, which is used as an include). You'll need to change a few varibales here and there to get it to work for you and you'll probably find a few of my own variables lurking in that text file; if you've any questions, let me know and I'll help you out.
