Check that the path to the avatar as specified in the admin Review/Edit Avatars page is correct

There seem to be a few mistakes in that file, I have to go to work now but I will look at this when I get home tonight, in the meantime feel free to go back and carefully re-check the changes you have made in that file with the changes the read-me "suggested" you made.
