Does anybody know if there is a program where you can create table structures (with relations etc) for example access or mysql?

Normally, with Access itself you can do it.
For MySQL take
phpMyAdmin http://forum.snitz.com/forum/topic.asp?ARCHIVE=true&TOPIC_ID=29776&SearchTerms=mySQL

or

MySQLMan http://www.gossamer-threads.com/scripts/mysqlman/

Visio, ERwin and other programs allow you to design db's graphically and then create the db structure. These are pretty pricey.

MS SQL Server can do database diagrams.

I use pen & paper for most of my design work :)

For creating tables i use phpMyadmin I was looking for a program that can design db's graphically. I think programs like MySQL-front cannot do that graphically?

You need the Visio Enterprise to actually create a database from you design, I think, and that's around 1K US$

You won't be able to create a database from your diagrams. You will be able to create a diagram from your database.

I have Visio Pro 2000 myself and seldom if ever use it for DB work since it doesn't make databases from it's diagrams.
